In April 2019, SPS Companies, Inc. will again offer free, on-site biometric screenings to all employees and spouses. Biometric screenings offer you two things:
- A clear health assessment, alerting you to any possible health concerns, and
- A financial savings opportunity related to your insurance deductible.
The information below will help you prepare for the upcoming screenings.

2019 Medical Insurance Premium Incentive Components
Although you are not required to participate, only employees and spouses who do so will receive the medical insurance premium incentive for 2020. The monthly incentive amounts can be found in the chart below. The incentives will show as a weekly credit on the first four pay checks of each month.
So, how do you earn those incentives? The 2019-20 Health Incentive Program will consist of two components:
- Biometric outcomes, and
- Tobacco usage.
Those employees and spouses who are tobacco free and pass 4 out of 6 biometric benchmarks will receive the full discounted medical insurance premium effective January 1, 2020. Similar to last year, those who don’t qualify will be given an opportunity to attain the full premium discount through participation in coaching programs provided by Lockton.

What is a Biometric Screening and why is it important?
The Biometric Screening is a short health exam that provides information about your risk for certain diseases and medical conditions. Knowing your numbers – the results of your Biometric Screening is the first step to healthy living because it helps you understand where you can take action to improve your health. The screening includes body measurements and a venipuncture to obtain a blood sample.
Who is eligible to participate in a Biometric Screening?
All employees and their legal spouse can participate even if they are not covered on the SPS Companies, Inc. medical plan. The Company wants to promote a healthy lifestyle for all employees and families.
How long does the screening take?
The screening should take 10-15 minutes for the full blood draw and measurements which consist of height, weight and blood pressure. Please remember to arrive on time.
Who is performing the on-site screenings?
The health screening provider is Clinical Reference Laboratory (CRL). CRL is a licensed national health screening company that specializes in health and wellness services.
How do I schedule my Biometric Screening appointment?
If you and/or your spouse would like to participate in the on-site biometric screening, please pre-register for your screening appointment with the HR Representative at your location. Please sign-up by the deadline posted at your location, because you will not be able to sign-up after the deadline.
How do I prepare for my Biometric Screening?
• Before your screening appointment you are required to fast for 10 hours.
• DO NOT eat or drink anything except water; black coffee and tea are also fine; however, do not use cream or sugar.
• DO take all medications as directed by your physician(s) as you normally would.
• DO drink plenty of water before your screening to stay hydrated.
• If you have a medical condition that does not allow you to fast for 10 hours, please follow your healthcare provider’s instructions.
• DO wear a loose-fitting shirt that allows you to roll up your sleeve for a blood pressure check.
Do I have to complete all the measurements/tests in the Biometric Screening?
Yes. You will need to complete all screening measurements to qualify for the premium incentive for 2020.
Will I have to pay for the screening?
It is FREE! There is no charge to you or your spouse for the on-site screening.
What if I cannot make it to my on-site screening appointment?
You must contact your HR Representative no later than eight (8) business days before your screening date for schedule changes or cancellations to avoid charges to the Company.
Can I go to my primary care physician to get my Biometric Screening done?
Yes, please have them complete the Physical Exam and Biometric Screening Physician form, including lab values and submit the requested documentation to Lockton Health Risk Solutions. Preventive physical exams and lab values from visits will be accepted no later than April 30, 2019. Reminder: The preventive physical exam and lab screenings are covered 100% by your SPS Companies, Inc. Cigna medical plan as long as they are within preventive guidelines.
What if I have already had my preventive physical exam and lab values with my physician this year?
You may submit those results to Lockton Health Risk Solutions by completing the Physical Exam and Biometric Screening Physician form, as long as your date of service occurred after September 1, 2018 and no later than April 30, 2019.

Who is Lockton Health Risk Solutions?
Lockton Health Risk Solutions is our Nurse Advocate Program that is partnering with CRL to review the information from your completed HRA and the results from your biometrics screening so they can provide you with information to help you understand your current health and potential risks. They will be the point of contact for the tobacco cessation program and coaching calls.
What if I am pregnant?
If you are pregnant, you are not required to participate in the wellness process. Please have your physician fax a note that includes your full name and date of birth and states that you are currently pregnant and under their care to Lockton Health Risk Solutions at Fax # 888-251-2264 by April 30, 2019.
What if I cannot participate in the biometric screening due to a health condition?
If you cannot participate due to a health condition, please contact Lockton Health Risk Solutions Member Support by emailing email@example.com or calling 888-251-2260 to request an accommodation form.
Is the screening done on work time?
Yes. Screenings will be held at the start of each shift and on Company time.
Are the test results confidential?
All Biometric Screening information is personal and confidential, as protected by federal law. SPS will not have access to your individual screening results. This information is to help you understand your health so you can take appropriate action to improve your health. SPS will only be given an aggregate report (no names or identifying information) which outlines the Company’s “overall health status” (i.e. total percent of employees with normal blood pressure) to help determine the health insurance needs and benefits of potential wellness programs.
Can I return to work afterward?
Yes. It is safe to return to work after your screening. Also, healthy snacks (fruits, vegetables, granola bars, etc.) will be provided to you immediately after your screening. If you want a heavier meal you will need to bring your own food.
Can I submit my biometric results from CRL for the wellness benefits under my Allstate Cancer and/or Critical Illness policies?
Yes. You can file a Wellness Benefit Claim for your biometric screening with Allstate Cancer and receive $100 per calendar year per insured individual and/or Allstate Critical Illness and receive $75 per calendar year per insured individual, if you or your spouse are currently covered under that policy. What if I have questions about the Biometric Screening event at my location?

When will I get my results?
A Results Letter will be mailed to each participant’s home address 7-10 days after the screening.
What if I do not meet 4 of the 6 benchmark criteria?
You will need to complete an online Health Risk Assessment (HRA) by May 31, 2019 if you submitted a physician form, as well as three Coaching calls with a nurse, with the first call being completed no later than June 30, 2019, the second by August 31, and the third by Sept. 30, 2019. An HRA is a health questionnaire, used to provide individuals with an evaluation of their health risks and quality of life. Here are the steps to complete that:
- You will log on to the Health Risk Solutions webpage and click the button “Take Health Risk Assessment,” then select your language preference. Or you can request a paper copy from your HR Representative to complete and then mail to Lockton Health Risk Solutions.
- Next, enter your employer ID #555017. At the time you begin the HRA, an email will be sent to you containing a 14-digit personal login. You will only need to use this in the event that you would need to stop the HRA and restart where you left off at a later time.
- If you completed the HRA in previous years, you will need to create a new username for 2019. Re-entering your 2018 HRA and making the changes for this year will not qualify as completion.
- If you do not have an email address, please use email@example.com when it asks you for your email address.
- Please allow 20-30 minutes to complete the HRA.
- Complete three Coaching Calls with a Lockton Health Risk Solutions nurse to set short-term goals and work toward accomplishing these goals.
What is a Tobacco Attestation form?
All employees and spouses who want to participate in the 2020 Wellness Program will need to complete this form. This is a form that you will complete confirming if you are a non-tobacco user or a tobacco user. This information will determine if you are eligible for the tobacco incentive for 2020.
If I want to participate in the 8-week tobacco cessation program how do I sign-up?
It is easy! On your Tobacco Attestation form you will need to check the box next to “I am interested in receiving information about tobacco cessation program”, and a nurse from Lockton Health Risk Solutions will contact you with information and instruct you on the steps that need to be taken. Be sure to check your email or voice mail on a regular basis for messages from Lockton Health Risk Solutions.
If I meet the outcomes and tobacco cessation criteria, but my spouse does not, will I receive a partial credit for the premium incentive?
No, if enrolling in Employee + Spouse or Family medical coverage BOTH the employee and spouse must complete all steps to receive the premium incentive for 2020.
Can I complete my HRA and Coaching Calls during work hours?
No, you will need to complete them both on your own time. Also, Lockton Health Risk Solutions will contact you via email or by phone to schedule a time that works best for your Coaching calls and establish a schedule from there.

You will receive a packet of information in the mail, which will include detailed information about the biometric screenings and the health incentive program. The packet will also include any forms you would need to participate.
