It might feel like biometric screenings happened a long time ago, but there are two steps you may need to complete by June 30 to ensure you can get your full discount.
Tobacco Attestation Form
All participants must complete and submit the Tobacco Attestation Form by June 30, 2019. (Click the link below to download.) This information will determine which tobacco insurance rate you receive. If you are a tobacco user, you can still earn the non-tobacco rate by completing the Tobacco Cessation Program. You can enroll by filling out the enrollment section at the bottom of your attestation form, and the nurse will contact you with information and instruct you on your next steps. The tobacco cessation program must be complete by September 30, 2019 to receive the non-tobacco rate.
Nurse Coaching Call
If you did not meet the 4 out of 6 benchmark criteria, you can still earn the full incentive. The first step was to complete the online health risk assessment (HRA) by May 31. If you did that, the next step is to complete three coaching calls with a Lockton nurse, who will be contacting you either by email or phone. The first call must be completed by June 30, 2019, so be on the lookout for their call or email!
Some more dates to remember beyond June are:
- August 30, 2019 – Submit the physician care plan form and complete the 2nd coaching call.
- September 30, 2019 – Complete the 3rd coaching call.
- September 30, 2019 – Deadline to complete the tobacco cessation program.
